Waiver

**If you have problems logging into the waiver system, please check back while we remedy the situation. The University gives all annual and Pharmacy students a deadline of 8/13/17 to enroll or waive.**

Waiver link for ALL FALL PROGRAMS will be available 7/15/17

Physician Assistant Program – The waiver deadline ended on 06/16/17 for the Physician Assistant Program.

The waiver deadline ended 7/13/2017. The online waiver system can no longer accept new waiver requests.

**Please read the waiver criteria below and then select the appropriate waiver link to submit your waiver.**

DOMESTIC STUDENTS WAIVER

All Domestic students enrolled in 3 or more credit hours and Psychology Graduate students enrolled in 1 or more credit hours are required to enroll in Pacific University’s Student Health Insurance Plan (SHIP) unless a waiver is submitted and approved. The cost of the insurance coverage is not included in your tuition or fees and will appear as an additional charge on your account.  Students can request a waiver to SHIP but must demonstrate that they have equivalent, alternative health insurance coverage.

To be eligible for a waiver of enrollment in the Student Health Insurance Plan, the University requires that students provide evidence of other comparable health insurance coverage.  Please be advised that the waiver request will be reviewed and verified active with the insurance carrier.  Notification of acceptance or rejection of this request will be sent to your University email within seven business days.

To qualify for a waiver, your health insurance plan must meet the requirements as listed below.

  1. Dates of coverage meet or exceed the requirement for the school semester.
  2. Have equivalent coverage to the University’s Student Health Insurance plan including, but not limited to:
    1. All benefits applicable in Oregon.  If you plan to, or your program includes, travel outside of the State of Oregon (for example: clinical rotations for graduate health profession students, club sports, activities), your insurance plan should include comparable coverage for all types of medical care that extends beyond Oregon borders;
    2. Full coverage for pre-existing conditions;
    3. Unlimited benefits;
    4. And be Affordable Care Act (PPACA) compliant (ask your provider if your plan is PPACA compliant to verify).

If your plan meets the above criteria and you want to waive enrollment from the Student Health Insurance Plan, please acquire an electronic copy of the front and back of your insurance ID card and then select the link below to submit your waiver request. Once you login, select the RED button under the “No, I do not want the insurance.” Section.

       INTERNATIONAL STUDENTS WAIVER

All international students are required to enroll in Pacific University’s Student Health Insurance Plan (SHIP) unless a waiver is submitted and approved. The cost of the insurance coverage is not included in your tuition or fees and will appear as an additional charge on your account. International students can request a waiver to SHIP but must demonstrate that they have alternative health insurance coverage meeting the criteria set forth below.

In order to be approved for a waiver of enrollment in the Student Health Insurance Plan, your alternative health coverage must meet or exceed the requirements as set forth below and be submitted by the waiver deadline date. Please be advised that the waiver request will be reviewed for compliance and verified active with the insurance carrier. Notification of acceptance or rejection of this request will be sent to your University email within seven business days.

A.     International Students with a J-1 Student Exchange Visa must meet all of the following minimum requirements:

1.       Medical benefits of at least $100,000 per accident or illness.

2.       A deductible of not greater than $500 per injury or illness. (A deductible is the amount that you need to pay per injury or illness before insurance will pay the remaining balance).

3.       Covered benefits paid at a minimum of 75%.

4.       Medical evacuation coverage amount is no less than $50,000.

5.       Repatriation coverage amount is no less than $25,000.

6.       Policy cannot exclude payment for any alcohol related injuries or any other unreasonable policy limitations.

7.       The policy must be underwritten by an insurance corporation with an A.M. Best rating of “A-“ or above, an Insurance Solvency International, Ltd. (ISI) rating of “A-1” or above, a Standards and Poor’s Claims Paying Ability rating of “A-“ or above, or a Weiss Research, Inc. rating of B+ or above. ( McGraw Hill Financial/Standard & Poor’s Claims paying ability rating of A- or above, or a Moody’s Investor Services rating of A3 or above is also acceptable). Alternatively, the policy must be backed by the full faith and credit of the government of the exchange visitor’s home country.

Note: Travel plans are not acceptable for waiver approval.

B.     All Other International Students must meet one of the following criteria:

1.       Student is sponsored by the government of the student’s home country and is provided through a plan equivalent to the SHIP or a letter from the government sponsor guaranteeing payment of all health care expenses.

2.       Student is enrolled in a US employer-provided group or US individual health plan that is equivalent to the SHIP.

C.     All International Students must meet the following criteria (applicable to both above groups):

1.       Dates of coverage meet or exceed the requirement for the school semester.

2.       If the alternate health plan does not include medical evacuation and repatriation, a rider must be purchased providing coverage no less than $50,000 for medical evacuation and no less than $25,000 for repatriation.

Note: Travel plans are not acceptable for waiver approval.

If your alternate coverage meets the above minimum requirements, submit electronic copies of the following documents with your online waiver request:

a.       A scanned copy of the front and back of your health insurance ID card indicating the student as a covered member.

b.       A scanned copy of your complete policy, including coverage amounts, exclusions, and limitations in English using US dollars.

c.       A scanned copy of your medical evacuation and repatriation coverage (if you have this coverage).
